Notes:

• Each registration will only be accepted upon receipt of full payment. Delegate will be informed via □ email OR □ post (please indicate preferred option on registration form)
• Each registration form can only be used for ONE delegate
• Registered delegates unable to attend will be given a 50% refund of the total fee paid, provided that a written request is received by the Secretariat before 25th May 2008. No refunds will be issued for cancellations after this date.
• All approved refunds will be processed and issued sixty (60) days after the end of the conference.
